Supporting Our Elderly Patients: The Keeping Well in the Home Program

At Walker Street Doctors, we believe that everyone deserves to live with dignity and independence in their own home for as long as possible. That's why we're proud to be active participants in the Keeping Well in the Home Program (KWIP), an innovative initiative designed to support our elderly patients and help them maintain their independence.

What is the Keeping Well in the Home Program?

The KWIP is a proactive and preventative approach to care for older people that aims to:

  • Identify patients at higher risk of hospitalization
  • Provide timely access to regular GP appointments
  • Enhance patient experience by supporting them to stay well and independent at home
  • Connect patients with appropriate community services and resources

 

How Does This Benefit You or Your Loved Ones?

If you or a family member are enrolled in the KWIP, you'll receive:

  • Personalized Care Plans: Our team will work with you to develop a comprehensive care plan tailored to your specific needs and health goals.
  • Proactive Health Monitoring: Regular check-ins with our KWIP Care Coordinator and healthcare team to catch potential issues before they become serious.
  • Priority Access: Dedicated appointment times for KWIP patients and more timely access to care when needed.
  • Post-Hospital Support: Prompt follow-up within 5 business days after any hospital discharge to ensure a smooth transition back home.
  • Geriatrician Support: When appropriate, we can connect you with specialized geriatric care through our partnerships with local health services.
  • Home Visits: For patients who find it difficult to come to the practice, we can arrange home visits when clinically necessary.
  • Connection to Community Services: Our team can help navigate and connect you with various support services available in Northern Sydney, including:
    • Geriatrician Outreach Services
    • Home Nursing Services
    • Chronic and Complex Care Services
    • Transport Services
    • Social Work Support
    • Healthy Lifestyle Programs

Who is Eligible?

The KWIP is designed for older patients who may:

  • Have multiple chronic health conditions
  • Have had recent hospital admissions
  • Be at risk of future hospitalizations
  • Benefit from additional support to maintain independence at home
